内容記述 | In 2004, the authors, Japan s National Demonstration Project(NDP)task force, developed a proposal for “Formulation of the NDP Best Practices for management of High-Alert medications.”the NDP Best Practices were safety guidelines developed in collaboration with 13 hospitals, and comprised of 16 best practices to prevent high-alert medication errors. At the time, the realization of every single one of them was believed to be extremely difficult but the NDP was a breakthrough project, meant to demonstrate that the challenges could be overcome. Between 2008-2012, Japan s 100,000 Lives Campaign(the“100 k Campaign”)was rolled out nationally to improve patient safety systems. the campaign was a joint enterprise in which medical institutions, healthcare professionals, medical associations, and healthcare-related academic societies nationwide overcame barriers between facilities, professions, and areas of expertise to work together to make patient safety targets a reality. the NDP Best Practices were adopted by the campaign for its“Action Plan goal #1: Preventing high-alert medication errors” and were implemented in the country s 282 most important hospitals. In this review, I will discuss the project s two preliminary studies(a fact-finding survey concerning the handling of high-alert medications and a survey of medical staff knowledge regarding high-alert medications)and the process of formulating the draft practices based on the results of these studies. In addition, we will describe the outcomes of the empirical studies conducted in the 13 participating hospitals, which were each allocated a few practices to implement and test. | |||||||||||
