At the time, the realization of every single one of them was believed to be extremely difficult but the NDP was a breakthrough project, meant to demonstrate that the challenges could be overcome. Between 2008-2012, Japan s 100,000 Lives Campaign（the“100 k Campaign”）was rolled out nationally to improve patient safety systems. the campaign was a joint enterprise in which medical institutions, healthcare professionals, medical associations, and healthcare-related academic societies nationwide overcame barriers between facilities, professions, and areas of expertise to work together to make patient safety targets a reality. the NDP Best Practices were adopted by the campaign for its“Action Plan goal #1: Preventing high-alert medication errors” and were implemented in the country s 282 most important hospitals. In this review, I will discuss the project s two preliminary studies（a fact-finding survey concerning the handling of high-alert medications and a survey of medical staff knowledge regarding high-alert medications）and the process of formulating the draft practices based on the results of these studies.
