A 42-year-old woman with adenomyosis developed motor aphasia with heavy menstrual bleeding and was admitted to our hospital. magnetic resonance imaging showed multiple hyperintense lesions in the left middle cerebral artery region on diffusion-weighted imaging, and magnetic resonance angiography showed occlusion of the left middle cerebral artery. Blood tests showed elevated CA125 and D-dimer and increased thyroid function. Screening for embolic sources and malignant tumors revealed no obvious abnormalities. Adenomyosis is known to cause hypercoagulability due to the production of CA125, which is a member of the mucin family of glycoproteins. It has also been pointed out that hyperthyroidism causes dysfunction of vascular endothelial cells, hypermetabolism and an increase in oxygen consumption of the brain, leading to cerebral ischemia. in this case, cerebral infarction was considered to be caused by the effect of hyperthyroidism on the hypercoagulable state caused by adenomyosis. Methods to prevent blood hypercoagulation associated with adenomyosis have not been established, and great care should be taken regarding complications that may cause cerebral ischemia such as hyperthyroidism.
